Impact of measurement and feedback on chlorhexidine gluconate bathing among intensive care unit patients: A multicenter study

Objective:
To assess whether measurement and feedback of chlorhexidine gluconate (CHG) skin concentrations can improve CHG bathing practice across multiple intensive care units (ICUs).
Design:A before-and-after quality improvement study measuring patient CHG skin concentrations during 6 point-prevalence surveys (3 surveys each during baseline and intervention periods).
Setting:The study was conducted across 7 geographically diverse ICUs with routine CHG bathing.
Participants:Adult patients in the medical ICU.
Methods:CHG skin concentrations were measured at the neck, axilla, and inguinal region using a semiquantitative colorimetric assay. Aggregate unit-level CHG skin concentration measurements from the baseline period and each intervention period survey were reported back to ICU leadership, which then used routine education and quality improvement activities to improve CHG bathing practice. We used multilevel linear models to assess the impact of intervention on CHG skin concentrations.
Results:We enrolled 681 (93%) of 736 eligible patients; 92% received a CHG bath prior to survey. At baseline, CHG skin concentrations were lowest on the neck, compared to axillary or inguinal regions (P < .001). CHG was not detected on 33% of necks, 19% of axillae, and 18% of inguinal regions (P < .001 for differences in body sites). During the intervention period, ICUs that used CHG-impregnated cloths had a 3-fold increase in patient CHG skin concentrations as compared to baseline (P < .001).
Conclusions:Routine CHG bathing performance in the ICU varied across multiple hospitals. Measurement and feedback of CHG skin concentrations can be an important tool to improve CHG bathing practice.

In September 2016, the annual meeting of the International Union for Quaternary Research’s Loess and Pedostratigraphy Focus Group, traditionally referred to as a LoessFest, met in Eau Claire, Wisconsin, USA. The 2016 LoessFest focused on “thin” loess deposits and loess transportation surfaces. This LoessFest included 75 registered participants from 10 countries. Almost half of the participants were from outside the United States, and 18 of the participants were students. This review is the introduction to the special issue for Quaternary Research that originated from presentations and discussions at the 2016 LoessFest. This introduction highlights current understanding and ongoing work on loess in various regions of the world and provides brief summaries of some of the current approaches/strategies used to study loess deposits.

Current procedural terminology codes generally do not distinguish between patients based on age or etiology of disease. As standardized payment schedules based upon such codes are being developed, the precise definition of a “typical” patient for each code becomes increasingly important. Since small size, young age, or presence of complex congenital disease could potentially render the work value of a physician's service different between children and adults, a study was jointly undertaken by the American College of Cardiology and the Cardiology Section of the American Academy of Pediatrics to compare relative work for the most commonly used Current Procedural Terminology codes in pediatric cardiology. A Technical Advisory Panel chose codes for evaluation and wrote clinical descriptors for each service. A separate rating panel of eight pediatric cardiologists (academic and private practice) and three adult cardiologists participated in a mail survey assigning work values to each service relative to current Health Care Financing Administration adult relative value units. The Panel then met and employed a modified Delphi process to arrive at consensus values. Of the 20 codes rated, only five were determined to be similar to adults. The pediatric median values were an average of 44% higher than the adult values (general cardiology codes 8% higher, echocardiography 90%, catheterization 49%, electrophysiology 35%). Thus, work values for cardiology were found to be different between children and adults in 75% of services assessed. The magnitude of these differences should be confirmed in a broader study. The Current Procedural Terminology Committee of the American Medical Association should consider, at a minimum, creating modifiers by specific age for certain procedures. Further study of the quantitative difference between services performed in adults and children is warranted.

The National Science Foundation-supported Materials Research Science and Engineering Center (MRSEC) on Nanostructured Materials and Interfaces at the University of Wisconsin – Madison has an extensive education and outreach effort. One theme of this effort is the development of instructional materials based on cutting-edge research in nanoscale science and engineering. The Nanoworld Cineplex contains movies and demonstrations that can be brought into classes, and the Nanotechnology Lab Manual contains numerous experiments that can be used for virtual or actual laboratories. Also available are kits, software, teaching modules and articles. A hands-on kit for nontechnical audiences, “Exploring the Nanoworld,” has been produced in collaboration with the Institute for Chemical Education.
In this paper, novel hands-on demonstrations and innovative laboratory experiments aimed at the college level will be highlighted. High-tech devices and materials such as light emitting diodes (LEDs), shape memory alloys, amorphous metal, and ferrofluids are discussed in the classroom and studied in the laboratory as illustrations of nanotechnology and its impact on energy, the environment and our quality of life. These examples illustrate interdisciplinary research that provides connections among materials science, chemistry, physics, engineering, and the life sciences. They also highlight the tools of nanotechnology, such as scanning probe microscopy, electron microscopy, x-ray diffraction, and chemical vapor deposition, which are associated with the preparation and characterization of nanostructured materials. Demonstrations of the incorporation of nanotechnology to teach fundamental materials science principles presented are summarized at http://www.mrsec.wisc.edu/edetc.

Summary
When massive economic restructuring forced William H. Whyte Jr's (1956) ‘organization man’ to walk permanently out of the front doors of the corporation, many of the assumptions of a traditional career path followed shortly behind. In the wake of his departure, people were left pondering how they would survive without a job for life, a corporate ‘road map’ to guide their destiny and the firm's hierarchy to define their status or place in society. Without employers’ orderly structures, external guides for action, and linear career paths, however, just what is the future of career? When ‘the career’ devolves to the level of the individual does free agency prevail and a form of ‘career anarchy’ result? Is there anything that can be concluded about careers if everyone seems to be doing their own thing?
We argue here that the answer is an emphatic ‘yes’. The new environment suggests a shift from pre-ordained and linear development to perpetually changing career paths and possibilities. As a result a shift from ‘bounded’ careers – prescribed by relatively stable organisational and occupational structures – to ‘boundaryless’ careers – where uncertainty and flexibility are the order of the day – is increasingly common. The concept of the boundaryless career is broadly based, and intended to reflect the emergent pace of economic change. It does not characterise any single career form, but rather ‘a range of possible forms that defy traditional employment assumptions’ (Arthur & Rousseau, 1996, p. 6).
The myriad interactions of a boundaryless career over a lifetime of work experiences can appear overwhelming. A complex web of relationships emerges from numerous exchanges among individuals, teams, and employers. The interplay may seem downright chaotic.

Metals and alloys with relatively low melting temperatures such as lead and lead-based alloys are considered in Russia as prospective matrices for encapsulation of spent nuclear fuel in containers in preparation for final disposal in underground repositories. Now lead and leadbased alloys are being used for conditioning spent sealed radioactive sources at radioactive waste disposal facilities.

Summary
Back in the early 1970s, there was no established “field” of careers in organization and management studies. Tim Hall recalls that it was fun when he and a small band of organizational scholars, who viewed themselves as being on the cutting edge of theory, who liked to work on somewhat offbeat topics, and who would rather challenge than defend the academic “establishment,” embarked on their career studies. Anyone interested in examining the long-term issues associated with working in organizations was really on his or her own to find useful theory and research in a number of relevant social science disciplines, such as psychology, sociology, political science, economics, and anthropology. To do good research, you had to scan and learn from a number of disciplines and you had to develop your own theoretical ideas. In organizational terms, the scholarly environment in which we operated was complex, dynamic, and nonroutine.
The down side of all this was that sometimes you wondered who your audience was. Many questions gnawed away at you: Is anyone out there interested in this “career stuff?” What journals would possibly publish papers on careers? Who is in my reference group? Everyone I trade papers with is in a psychology, sociology, or education department. Yet I work in a business school. Am I in the right place? But you had a sense that it was all new. And you knew it was important as you saw more and more evidence of the long-term influence of careers on individuals, the institutions in which they worked, and the societies to which they belonged. There was a sense of being on the “ground floor” of something important.
